Description Form Search
Select Form Search if you want to search just by filling
in the form. You can use alphabetical characters, spaces, periods, and apostrophes, but not
wildcards (! or *) or connectors (AND, OR, and W/2) in your search.
SSN
You can use a Social Security Number as a search term. However, if you do not possess the
proper authorization, such as a member of a recognized law enforcement agency, it will not
appear in any of the retrieved information.

First Name
Enter the person's first name or initial. Middle Name / Initial
Enter the person's middle name or initial. Last Name
Enter the person's last name. Street Address
Enter a complete or partial street address.
Example:

City
Enter the city of residence for the person you are looking for. Your search will find results
for the exact city name as well as for associated city names.
Example: If you enter
Skokie (and
Illinois for the state)
your search will find records for Chicago as well as for Skokie.
State
Select the state the person resides in or select All Available States .
Zip Code
You can enter a zip code to limit your search to a
specific geographical area. We recommend you limit your entry to the 5-digit form most commonly
used.
Date of Birth
You can use a birth date to narrow your search. Searching on date of birth alone will not
produce any results; you must enter a last name or a street address in addition to the birth
date.
Enter two digits for the month, two digits for the day and four digits for the year,
separated by slashes.
Example: 01/05/1970
Company / Organization Name
You can enter a Company/Organization Name specific to
the provider.
License ID
Enter the license number. License State
Select the state in which the license was issued from
the drop-down list or select "All Available States".
Tax ID
Enter the taxpayer ID. National Provider Identifier
Enter the National Provider Identifier specific to the
provider.
Search
Click the Search button
to begin your search.
Clear Form
Click the Clear Form link
to clear all the data you have entered in the fields.
